Nighttime accidents--help!

Our 4mos old Tuggy has started a very frustrating habit--he is having accidents every night in his room, no matter how often we get up to let him out. We give him dinner around 6pm every day and limit his water after that, always take him out at about 11pm and then will take him out around 3. Seems like it should be sufficient, but for the past week or so, we have been greeted by a couple of pee spots and poop every single night!! What Type of Doodle(s) do you have? What are their names? Tell us about your doodle(s) or any other pets you have!
We added "Tug" to our family in August, and he is a joy! He is a 4mos. old retriever/standard red poodle cross.
Where did you get your doodle(s)?
Breeder
Why did you choose a doodle rather than another breed or mix?
I did a lot of research before we settled on a Goldendoodle--and I was won over by the intelligence, gentleness and family friendly nature of the doodle. We have two small children, and already Tuggy is great with them.
What are Your Favorite Things to Do With Your Doodles?
Lots of fetch, playing "tugaroo" and going for walks.
